Individual Health Care Plans (IHCP) are designed to keep children with diagnosed medical conditions safe, well and involved at school. It is a written document that specifies your child’s diagnosed medical and mental health condition and the help that the school can provide for your child and what to do if there is a medical emergency.
IHCPs are kept confidentially in school and are only shared with those who support your child.
It is very important that our school is advised of a child’s medical or mental health condition to ensure that we hold the right information to support your child.
The IHCP will be reviewed yearly; unless parents/carers or professionals advise of any medical changes within the year.
